We are sure you will agree that it is ethical and imperative to maintain proper test security to protect professionals and end users, and therefore it is essential for us, as distributors, to prevent the sale or disclosure of tests and assessments to people who are not trained in their use. We therefore require confirmation of qualification level for use of high-level tests and will sell these only to individuals who provide credentials that meet these standards.
We kindly request declaration of your qualification level in order to purchase your selected assessment tool. All fields must be completed, otherwise we are not authorised, as the distributor for this assessment tool, to release it for sale.

Tests may be purchased by individuals with:
A master’s degree in Psychology, Education, Occupational Therapy, Social Work, Counselling or a field closely related to the intended use of the assessment, and formal training in the ethical administration, scoring and interpretation of clinical assessments. A full list has been declared in the “Profession” section of the form by the creator of the assessment tool.
OR
Certification by, or full active membership of a professional organisation (e.g. RcSLT, RcOT, BERA, BMA etc) that requires training and experience in the relevant area of assessment.
OR
A degree or license to practice in the Healthcare or Allied Healthcare field.
OR
Formal, supervised mental health, Speech and Language Therapy, Occupational Therapy, Social Work, Counselling, and/or educational training specific to assessing children or in infant and child development, and formal training in the ethical administration, scoring and interpretation of clinical assessments.
By filling in this form you certify that:- You will update this information if it changes.
- You are qualified to properly use any test and assessment products you order and you have provided Winslow Resources/ROMPA with only accurate and true qualification information.
- Any test and assessment products purchased under your account will be used by you and/or under your supervision.
- Any test and assessment products purchased under your account will be used in accordance with all applicable legal and ethical guidelines .
- You have read and hereby agree to the Qualification Terms and Conditions of Sale and Use and you agree that you will not resell any test or assessment product.
- You understand that violation of any Qualification Terms and Conditions of Sale and Use may result in the revocation of my right to purchase as a qualified customer. If there are any changes that may affect my qualification to purchase, you will immediately notify Winslow Resources/ROMPA of such changes.
